A leading organization that supports post-stroke rehabilitation needs, is currently seeking an experienced candidate to join its team as an Occupational Therapist.
Job Responsibilities
- Conduct occupational therapy specific assessment for clients during admission, periodic care plan reviews and discharge
- Provide effective occupational therapy sessions and develop innovative solutions to meet the client’s needs
- Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making
- Document therapy plans, assessment results, goals and outcome measures according to the organization’s guidelines
- Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholders
- Ensure proper use of technological-assisted rehabilitation and train clients in the use of assistive device
- Formulate discharge and continuity of care plans based on clients’ performance in consultation with relevant stakeholders
- Facilitate community integration through collaboration with other community partners as required
- Participate in case discussion with the care team
- Ensure documentations related to clients’ care are accurate and completed timely to reflect care and outcomes
- Assist Clinical Lead in the execution, development, and review of clinical program and services
- Support in the training and supervision of Therapy Assistants
Job Requirements
- Possess minimum Diploma/ Degree in Occupational Therapy
- Must be AHPC Full Registration
- Possess at least 2 years’ relevant experience in neuro rehabilitation
